Acne is an inflammatory change of skin appendages such as sebaceous glands and hair follicles. Excessive sebum production causes the sebaceous glands to become clogged. Most forms of acne are endogenous, meaning that the cause of acne comes from within. Usually, acne begins with hormonal changes. The most common endogenous form is acne vulgaris, also known as "common acne." Common acne usually appears during puberty and ends around the age of 25.

What happens with acne

Acne begins with a preliminary stage that is not yet visible - the so-called microcomedones. Microcomedones are so small that they cannot be seen with the naked eye, but can be detected microscopically. This preliminary stage leads to the proliferation of horn-forming cells. In acne patients, this step is accompanied by micro-inflammation. This is how the first possible symptoms of visible acne develop. Micro-inflammations are considered to be one of the main causes of acne. There are several causes for the actual development of visible inflammation, the most common being hormonal changes, environmental influences or the use of certain medications. Excessive sebum production is the basis for the growth of bacteria and thus for the development of the visible pimples.

Difference acne and blemished skin

Unlike acne, blemished skin has isolated pimples and slightly red areas. Blemished skin can usually be treated with light skin care products. Pimples or reddened areas in blemished skin are caused by dirty or clogged pores. Acne, on the other hand, is a chronic inflammatory disease of the sebaceous glands that can also be divided into different stages. Acne is more than just individual pimples or reddened areas.

The 4 stages of acne

Acne can basically be divided into four stages. Each stage depends on the severity of the acne.

  • Acne comedonica

    This is the mildest form of common acne. Acne comedonica involves blackheads, which may be open or closed. The blackheads form only on the face and scarring is usually minor.

  • Acne papulopustulosa

    These are blackheads and pimples that can form on the face and back. In this intermediate form of acne, scarring may occur due to inflammatory processes.

  • Acne conglobata

    This is the most severe form of acne. It appears on the face and back in the form of pustules, nodules and pimples. In the third stage, abscesses and fistula formation are also possible. Patients may also suffer from significant scarring.

  • Acne fulminans
    This form of acne, also known as acne maligna, is very rare. The course of acne fulminans is comparable to acne conglobata, but fever, joint inflammation and leukocytosis may also occur.
